    Donating 1.5% of your tax to the „Nadzieja Na Przyszłość” Foundation via PIT-28 is very simple and can be done either in paper form or electronically. Please note that providing an incorrect KRS number or amount may prevent the donation of 1.5% of your tax to the organization. A. Paper Version: • Download the PIT-28 form from the Tax Office or the Ministry of Finance website. • Fill out the PIT-28 form. • Navigate to Section S: „Request to allocate 1.5% of the due tax to a public benefit organization (OPP).” • In the field „KRS Number” (position 251), enter the KRS number for the „Nadzieja Na Przyszłość” Foundation: 0000919536. • In position 252, enter the amount you wish to donate.
